Requirements

You will need to complete the Privacy Act waiver form in order for my office to make inquiries on behalf of your request. Be sure to sign the form and include your address and daytime phone number. Return the waiver along with any additional information pertaining to your case to one of my three state offices nearest you.

The signature of any spouse, child, or person designated as the Power of Attorney for a living veteran will not be accepted by the Veterans Administration, unless they are designated as the veteran’s fiduciary. Please check with one of my offices if you are unsure about whether or not this applies to you.
