Be sure to include copies of pertinent documents such as the applicant's "Receipt Notice" and "Notice of Action." Note that the form can only be completed by the Petitioner or the Beneficiary. Upon receipt of your completed form, my office will conduct an inquiry with the USCIS, on your behalf, to determine the status of your application.

Unfortunately, USCIS is experiencing a large influx of applications and is therefore unable to respond immediately to our inquiries. Please allow 12 weeks for processing. We will notify you by letter or telephone as soon as we receive a response from USCIS. I thank you in advance for your patience and cooperation.
