Please know that the Privacy Act of 1974 requires that I have formal authorization from you before making a congressional inquiry to a federal agency on your behalf. This law was enacted to protect individuals from unauthorized disclosure of personal information without their consent. In order for us to serve you, please print out and fill Privacy Authorization Form and then fax or mail it to my office.

When returning the privacy waiver, it would also be very helpful if you would include all pertinent information such as your name(s), mailing address, telephone number(s), Social Security Number, Medicare number, Alien number, case number, etc. Also please include documentation from the agency including correspondence, bills, and other information relevant to your case. It is also most helpful if you include a letter describing the situation and explaining your request. Please indicate if it is acceptable to forward your letter to the relevant agency.

If you are contacting me regarding a legal issue:
The rules of the House of Representatives prevent me from intervening in the proceedings of any court of law (including appeal proceedings), since these are activities of the judicial branch of government. If you are having a legal problem involving the courts, you may need to consider help from an attorney.
