The Privacy Act of 1974 (Public Law 93-579) prohibits federal and state government agencies from releasing any information or discussing anything regarding another individual without the individual's written permission. Regardless of which agency you need help with, in order to comply with the provisions of the Privacy Act, a constituent requesting help will need to complete a privacy authorization form.

Steps to Begin Federal Casework

  1. Download the Privacy Act Form (in .PDF Format).
  2. Fill out and sign the Privacy Act Form.
  3. Fax the Privacy Act Form back to the Palm Harbor Office at (727) 784-6471.
  4. After receiving the signed Privacy Act Form, a caseworker will be in touch with you.
