State Department/Passports Information & Requirements
The U.S. Department of State represents and protects American interests around the world. The agency issues passports and assists Americans with travel abroad. Its website travel.state.gov provides information regarding passport requirements, travel safety announcements and explains entry requirements and services available to Americans living or traveling abroad.
Requirements
If you are experiencing a federal government agency, please complete, print, and sign the Privacy Act waiver (available at Step Two). You may fax or mail the completed form with any supporting documentation to my office location that serves your area.
