assistance follow these easy guidelines:
- Confirm you live in Georgia’s 4th Congressional District

- Write us a letter requesting specific assistance including details
about the matter.
- Attach copies of all relevant documents or correspondence that
you have sent to and received from the agency involved.
- Thoroughly complete and sign a Privacy Release Form. (Word

district office nearest you.
The Privacy Act of 1974 requires that Members of Congress and their
staff have written authorization before they can obtain information
about an individual's case.
As a matter of Congressional courtesy, each Member of Congress only
serves his or her own constituents and will refer those residing outside
their Congressional District to the appropriate Representative.
While we cannot ask the agency to do anything it would not normally
do under its regulations, we can assist you in working with the agency
to ensure that your rights are respected and that procedural errors
are corrected. Please remember that staff assistance is available
to help you with federal matters only.
Because of the Constitutional separation of powers between the three
branches of government, judicial matters do not fall under Congressional
jurisdiction.
Judicial issues might include child custody, divorce, criminal trials
or imprisonment.
Problems which involve State of Georgia agencies are also not under
Congressional jurisdiction. Contact the Governor's Office for assistance

State issues might include child support, motor vehicle registration
or loss of license, public assistance or state taxes.

Service Academy Appointments

As your Representative in the U.S. House, each year I have the privilege
of nominating a limited number of people to four of the five service
academies. The honor of attending a service academy comes with an
obligation and commitment to serve in the military for a minimum of
five years upon graduation. Best wishes to you as you pursue your
goal of attending a U.S. Service Academy and serving our country.
What is a nomination?
Members of Congress may nominate applicants who meet the eligibility
requirements established by law. A candidate may seek a nomination
from the following:
• A member of the U.S. House of Representatives who represents
the congressional district in which the candidate resides
• Both U.S. Senators from his or her state
• Apply directly to the Vice-President of the United States
What are the basic eligibility requirements?
Each applicant for a nomination must meet the following eligibility
requirements as of July 1st of the year of admission to an academy:
• Age: Be at least 17 years old, but not have passed the 23rd
birthday
• Citizenship: Must be a U.S. citizen
• Marital Status: Must be unmarried, not pregnant, and without
legal obligation to support children or other dependents
• Residence: Must reside within the boundaries of our Congressional
District
• Skill / Fitness: Must meet the medical, physical, and academic
requirements of the Academy.

What must be included in my nomination file?
A complete nomination file consists of the following items:
• Application form
• Current photo
• Official high school transcript
• Official College Board Scholastic Assessment Test (SAT) results
(Code: 4809)
• Activities questionnaire
• Essay as outlined in application (500 words or less)
• Applicant contract
• Affidavit of domicile
• Principal/Guidance Counselor form
• Three letters of recommendation: math teacher, science teacher,
personal (non-related adult)
What is the deadline to apply for a nomination from me?
Your complete nomination file and application must be received in
my Tucker, Georgia office no later than 5:00 pm, Monday, October 27,
2008 to receive consideration for nomination. Partial applications
are not considered and no exceptions will be made. Please do not submit
your file until it is complete. Only SAT scores sent from the College
Board will be accepted separately.
All applicants who submit complete nomination packages by the deadline
and have an open file with the service academies of their choice will
be scheduled for an interview with my Service Academy Review Board.
Each eligible applicant will be notified in writing of the date and
time for an interview.

How many academies are there?
There are five service academies.
Three reside within the Department of Defense:
• U.S. Military Academy at West Point, New York
• U.S. Naval Academy at Annapolis, Maryland
• U.S. Air Force Academy at Colorado Springs
One resides within the Department of Transportation:
• U.S. Merchant Marine Academy at Kings Point, New York
One resides within the Department of Homeland Security:
• U.S. Coast Guard Academy at New London, Connecticut
Are the requirements the same for all of the service academies?
No. Although many of the qualifications are similar, each academy
operates under its own admissions guidelines. Visit the academy of
your choice via the web sites noted for each academy's particular
admissions qualifications.
It is not necessary to seek a nomination to the Coast Guard Academy.
U.S.C.G.A. accepts candidates based on their own testing criteria.
Who should I ask to write a personal letter of recommendation
on my behalf?
Some of the people applicants typically seek recommendations from
include:
• Teachers
• Counselors
• Employer
• Coaches
• Clergy
• Scout Leaders
Will I automatically be selected by the academy after I receive a
nomination?
No. You will be placed among a select group of nominees, and the
academy of your choice will make the final decision. If selected,
you will receive an offer of admission (an appointment) directly from
the academy.
What if I choose more than one academy? Will I be limited
to a single nomination?
No. You can be nominated to multiple academies depending on how
well you have competed among this year's pool of candidates. It is
also possible to win multiple appointments.
