Where Do I Apply?

Many post offices, clerks of court, public libraries and other state, county, township, and municipal government offices accept passport applications. To find out where you can apply, visit the State Department's passport office search feature.

Where Can I Find Passport Forms?

You can download passport forms, or call the National Passport Information Center at 1-900-225-5674.

What Do I Need To Bring When Applying?

  • Completed passport form
  • Proof of U.S. citizenship — either a certified copy of your birth certificate or a previous passport.
  • A set of identical recent photographs (2x2 inches/must show full face, front view)
  • Proof of identity — valid driver’s license, military ID, government ID, corporate ID, certificate of naturalization or citizenship
  • Social Security card (a social security number is not required to get a passport)
  • Check or Money order made payable to Passport Services. Cost is $60 for a 10-year passport if you are over 16 years of age. This includes $45 plus the $15 notary fee. For those under 15, cost is $40 for a 5-year passport. Expedited passports (those needed in under two weeks) will cost an extra $35 and proof of why you need it expedited must be provided.
