The Privacy Act of 1974 requires that Members of Congress and their staff have written authorization before they can obtain information about an individual's case. Please print out and complete the Privacy Act Release form (PDF, download PDF reader) and return to one of my district offices before we inquire about your claim. Also, copies of any documents or correspondence that you may have from the agency involved may be helpful.
