How our office can help

 

If you are facing a problem with a federal government agency (such as the Bureau of Citizenship and Immigration Services, IRS, Social Security, Veterans Affairs, or any other federal agency), please contact my district office.

The Privacy Act of 1974 requires that Members of Congress and their staff have written authorization before they can obtain information from any government agency about an individual's case.

You can either download this Authorization Form (PDF) from our website or my office can send you a copy of this form. For casework related to immigration, please use the Immigration Casework Authorization form (MS Word .doc). You must fully complete this form and send it in with copies of all pertinent information relating to your case, including a detailed letter explaining your problem and how you would like us to assist you.

Please know that we can only assist with Federal issues. For problems relating to a city or state agency, please contact your local representative. Thank you and we look forward to assisting you.
