Because of federal law, in many cases you will be required to submit a consent form prior to requesting my assistance.

Please chose a format for the form:

PDF Version - This form is in Adobe Acrobat format and you can complete the form on-screen with Acrobat. However, you still must print the form and send it to us.  You must have Adobe Acrobat Reader installed to view a PDF file.

HTML Version - This form is in HTML format, and can be viewed with any web browser.  To use this version, print the form, fill it in by hand, and send it to us.
