As one of your federal representatives, I may be able to assist you in
resolving a problem with a federal agency. My staff makes every effort to
assist Pennsylvanians with a variety of issues; however, there are some
matters over which I have no jurisdiction.
I am able to provide assistance to you if:
- You are a resident
of the 12th Congressional District, AND
- Your problem is with a
federal government agency.
Please keep in mind that I cannot force a federal agency to decide an issue
in your favor nor to overturn a final decision. If you are seeking
assistance with an issue that
involves local or state government,
you should contact your Pennsylvania State Senator or Representative.
If you are seeking assistance with a matter involving a lawsuit or litigation,
my staff and I are prohibited from giving legal advice or intervening in legal

What my staff and I need from you:
Once you have determined that your case involves a federal agency, you will
need to submit the following information to enable my staff to review your case
and contact the appropriate federal agencies on your behalf.
1. A clear and concise letter describing the matter about which you request
help, including a daytime phone number and address in case my staff needs
additional information from you.
2. Also, be sure to include any identification numbers such as your social
security number, date of birth and case or file numbers.
3. A completed Privacy Act Release Form to grant
your written consent for my office to contact a federal agency on your
behalf.
You should submit this information to either of my district offices via fax
or mail at the numbers and addresses listed below.
