Flag Request Form (PDF)

I am glad to provide you with a flag flown over the U.S. Capitol.

Due to the numerous requests received, my office is unable to provide these flags free of charge. We ask you to reimburse us for the cost of the flag, the charge for flying and mailing it.

Please complete the Flag Request Form and mail it with payment to my office:

Important: Please mail completed form along with a check made out to "Charlie Melancon's Office Supply Account." Normal processing time for flags flown over the Capitol is four to six weeks. If you are simply ordering a flag without having it flown, requests can be expedited.

If a rush order is needed, please indicate the date the flag is needed by, and we will try to accommodate your request. All items are at cost; no profit is incurred.
