Federal Agencies
Congresswoman Bono Mack can help constituents with difficulties they may have encountered when dealing with federal agencies. Under the Privacy Act of 1974, individuals needing assistance must complete this authorization form and return it to a district office, along with any other documents that may help resolve their case. (Please send only copies of original documents.)

After we have reviewed the completed forms, we will contact the agency in question on your behalf.

According to the U.S. House of Representative Committee on Standards of Official Conduct, Congresswoman Bono Mack cannot act as an attorney or offer legal assistance in any way.  Congresswoman Bono Mack's jurisdiction as a United States Congresswoman does not extend to programs and other matters governed by the departments of the State of California.
