In emergency situations Congresswoman Bono Mack can help speed passport applications and provide travel information from U.S. embassies abroad.
Many outlets such as post offices, clerks of court, public libraries and state or municipal government offices accept passport applications. More information on where to apply for a passport is available from the State Department's passport office search feature.
Passport forms may be downloaded from the State Department Web site. Forms may also be obtained by calling the National Passport Information Center at 1-900-225-5674.
Reminder: When applying for a passport, applicants should bring:
Completed passport form;
Proof of U.S. citizenship—either a certified copy of your birth certificate or a previous passport;
A set of identical recent photographs (2x2 inches/must show full face, front view)—many offices will also take photographs for a fee;
Proof of identity—valid driver's license, military ID, government ID, corporate ID, certificate of naturalization or citizenship;
Social Security card (a social security number is not required to get a passport);
Check or money order made payable to Passport Services. Age 16 and older: The passport fee is $55. The execution fee is $30. The total is $85. Under Age 16: The passport fee is $40. The execution fee is $30. The total is $70.*
Expedited passports (those needed in under two weeks) will cost, in addition to regular application fees, $60 per application plus overnight delivery costs.*
