Academy Nominations Checklist

Please forward the following items to Congressman Tim Johnson's office no later than November 15th:

1. Application for Congressional Nomination to a U.S. Service Academy.
   
   
2. A copy of your official high school transcripts.
   
   
3. All Letters of Recommendation made on your behalf.
   
   
4. SAT & ACT Results (official results only please).
   
   
5. A small picture of yourself.
   
   
6. Any other materials that you think would be relevant to your application concerning your status in school, any clubs or organizations you may belong to, as well as sports and academic awards.
