3x5Nylon - $9.00
3x5 Cotton - $9.25
4x6 Nylon - $13.50
5x8 Nylon - -$18.00
5x8 Cotton - $20.00
The House of Representatives has added a $4.05 fee to cover the cost of flying flags over the Capitol. If you wish the flag to be flown, please indicate this on the enclosed form and note the date you wish it to be flown. You may also include any special message to be included on the accompanying certificate.
Please make your check payable to Stationary Account #MI-12-31 and write "flag reimbursement" on the check's memo line.
Allow eight to ten weeks for delivery and please send your check along with the completed form to:
