As many of you know, the Medicare prescription drug benefit program got underway on January 1. In the first few weeks since the program's inception, many beneficiaries have experienced very specific difficulties, including medications being dropped from their chosen providers, unexplained co-payment increases, or failure to receive a personal identification card from their plan provider. If you have encountered any of these or other types of problems, I encourage you to fill out this online form so that I may use your experience and information to adequately address any inconsistencies in this new program.

I will be using information collected from this form and from other communications in order to work with the Centers For Medicare & Medicaid Services and the House Energy and Commerce Committee, on which I serve, to address program discrepancies.
