Today marks one month since Medicare Part D plans were opened for enrollment. The Centers for Medicare and Medicaid Services is reporting that only 500,000 of 40 million eligible seniors have signed up so far. This low participation number is not surprising. After all, the fledgling program has been plagued by mishaps and misinformation. With two weeks left before drug coverage is scheduled to take effect on January 1, several serious implementation problems are frustrating seniors’ efforts to select and enroll in a plan.

“Today, Maryland seniors began the arduous process of trying to figure out whether to enroll in Medicare Part D and which of 47 different private prescription drug plans might be best for them. This morning, Marylanders searching the Medicare web site with the assistance of a few dozen knowledgeable volunteers at a Baltimore County Department of Aging event grew distressed and discouraged. The medicare.gov web site crashed often. Those who did get through found that the site still lacks complete information about participating pharmacies, covered medicines, and copayments. Many found that the popular brand name drugs they need are not being offered under the more affordable plan options. It is an outrage that Maryland seniors would be expected to make such an important and complex decision without all the necessary facts.

Maryland seniors are facing an overwhelming array of choices: 19 different companies offering 47 different plans with monthly premiums that vary from $6.44 to $68.91, including a Medicare managed care option called MedicareAdvantage.

At the July 21 hearing of the Ways and Means Health Subcommittee, I was encouraged that Chairwoman Johnson acknowledged the need to eliminate the Sustainable Growth Rate (SGR). Whatever reimbursement system this Congress enacts as a replacement for SGR must fully recognize the costs of providing care; if it does not, Medicare beneficiaries will continue to face diminished access to care.

The VA hopes this strategy will lower its pharmacy expenditures because drug companies often base their prices on the number of pills rather than the dosage. This means that the VA can save money by purchasing fewer tablets at stronger dosages. For our nation's elderly veterans, this policy is penny-wise and pound-foolish.
