Ordering Flags

Congressman Matheson’s office can arrange to have an American flag flown over the U.S. Capitol in honor of an individual, a group, or even to commemorate a special occasion.

Flag requests usually take 6-8 weeks to process and will be sent to you via U.S. Priority Mail.  Since mail to Washington, D.C. takes approximately 2-3 weeks to arrive, please send all completed flag forms and checks to the Salt Lake office to ensure timely delivery.  Every effort will be made to honor your request.  

Instructions
1)  Completely fill out the flag form (below).  Be sure to include contact information and any special instructions for your order (such as whether it is needed by a specific date).

2)  Print two copies of the form and keep one for your records.

3)  Mail a copy of the form, along with a check made out to "Congressman Matheson's Office Supply Account," to the district office.

4)  Please call the Washington, D.C. office if you have not received confirmation of your order within 3-4 weeks.
