District Office Services
One of the services Members of Congress provide their constituents
is assistance with federal agencies. Although Members of Congress
cannot guarantee a particular outcome, my office will work to help
you receive a fair and timely response to your problem.
Federal agencies often provide information and materials that can
solve your problem online. Please consult the website for the appropriate
agency before contacting my district staff for assistance.
Federal Agencies
My staff can offer assistance in the following federal issue areas.

Social Security, SSI, Medicare
- To replace a missing or stolen Social Security or Medicare card,
apply for Social Security/SSI Disability benefits or to locate
a Social Security office near you, please call 1-800-772-1213
or visit the Social Security website. http://www.ssa.gov.
- Congressman Kirk’s staff can obtain the status of pending
cases, expedite answers to questions, and trace missing checks.
In addition, the staff often assists constituents who are having
difficulty obtaining reimbursement of a Medicare claim, locating
a physician who accepts assignment, or determining which of constituent
health care plan is considered primary for the processing of medical
claims.

Immigration, Citizenship, Visa and Passport
Services

- The Bureau of Consular Affairs can assist constituents with
a number of travel, passport, and visa related issues. Please
visit the Bureau of Consular Affairs website for more information.

- For passport assistance, the Chicago Passport Agency is open
by appointment only. The automated appointment system permits
customers to schedule appointments from any touch tone phone.
Only customers who are traveling within three weeks are allowed
to schedule an appointment. Those traveling in more than three
weeks must apply for passports through the local passport acceptance
facility. Instructions for scheduling an appointment are in English
and Spanish. Schedule an appointment with the Chicago Passport

- Various post offices also accept passport applications. Please

- Constituents may contact my district office with a variety of
immigration issues including assistance with the immigrant visa
petition process and families adopting a child born abroad. U.S.
citizens and residents who have difficulty obtaining a passport
or other needed travel documents, and those experiencing problems
traveling abroad may also contact my district office for assistance.

Veterans
- Veterans may contact the Department of Veterans Affairs (VA)
regarding eligibility for benefits, disability claims, hospitalization
issues and appeals of VA decisions. The VA can be contacted directly

- My staff can assist veterans with applications for medical records,
service records, and investigation of their claims for benefits,
including disability. Veterans in the Tenth District can also

Military Personnel
- Active military personnel must first seek to rectify problems
through proper chain of command in the appropriate branch of the
Armed Services. The Department of Defense website provides active

- For customer service inquires and to expedite any service related

- Although the USPS has been an independent agency since 1970,
please contact my office if you are experiencing persistent problems
with mail delivery. Please send my staff a letter of complaint
with a detailed description of your problem and your address including
zip code.

Student Loans
- Answers to Frequently Asked Questions (FAQ's) about federal
student loans and federal student loan applications are available

the status of federal student loans during the application and
repayment process.

Internal Revenue Service
- The Internal Revenue Service can provide constituents with information,
publications, and tax payer assistance for personal, business,
retirement, and non-profit related tax questions. Please visit

- Constituents may seek assistance from my staff about federal
income tax documents for personal or business income. The District
Office staff also assists people by expediting a response when
problems arise regarding tax returns, delayed, lost or stolen
refund checks, or exercising the right to appeal a tax decision
by the IRS. The staff works to clarify misunderstandings between
the agency and the taxpayer to ensure that the taxpayer's position
is properly considered.
