My offices in Washington and New Hampshire act on behalf of my constituents in resolving problems with federal agencies. In order to protect your privacy, the law requires me to obtain written permission from a constituent before making inquiries on his or her behalf. Please print the authorization form and then fill it out. After I receive your authorization form, I will contact the appropriate agency regarding your concern.
