Sometimes dealing with the federal bureaucracy can be a challenge. I can help when you run into problems on federal issues like Social Security, Medicare, Veterans Administration, Small Business Administration, the Immigration and Naturalization Service, and the I.R.S. Members of my Texas staff are nearly all bi-lingual. In order for my office to assist you, please explain the nature of your problem and what you would like me to do. Please include the information below in a letter, or print and complete this form (PDF version):

  • Full name
  • Address
  • Home phone number
  • Daytime phone number
  • Date of Birth
  • Social Security number (If your case involves the Social Security Administration or the I.R.S.)
  • VA claim number and a copy of a DD-214 (If your case involves the Department of Veterans Affairs)
  • Alien resident number, recipient number, and current immigration status (If your case involves the INS)

Please include any other documents or correspondence that you think may be helpful to my office in pursuing your case. OR print out this form, SIGN IT, and return it to the office closest to you (see addresses above).
