- Full name
- Address
- Home phone number
- Daytime phone number
- Date of Birth
- Social Security number (If your case involves the Social Security Administration or the I.R.S.)
- VA claim number and a copy of a DD-214 (If your case involves the Department of Veterans Affairs)
- Alien resident number, recipient number, and current immigration status (If your case involves the INS)
