The Privacy Act of 1974 prohibits federal government agencies and departments from releasing personal information without the written permission of the individual involved. Once you have completed and signed the form, return it to my state office in your area. This provides the necessary authorization for me to follow up with appropriate government officials.
Please send a detailed letter describing your problem, the government agency involved and any other pertinent information related to your case. In order to assist you, your letter must include your Social Security number and signature. All letters regarding military service must be written by the person in the service.
