Icon for Obtaining a U.S. Passport

Many constituents travel outside of our nation each day and seek assistance from my office. Because travel plans may not be predictable, I am pleased to provide you with information that will help you.

In order to apply for a United States Passport, you must be a United States resident, either by birth or naturalization. In addition, you will need to obtain two passport-sized photos and take a photo identification card with you when you apply. To determine what is acceptable information to submit to the Department of State, please click here . If you are submitting a birth certificate as proof of citizenship, the document must be a certified copy and be issued by a state or county agency-not a hospital. If you need to obtain a copy of your birth certificate, contact the Department of Health in the county or State in which you were born. For your convenience, I am providing health department information in Ohio’s 9th District:

If you find that your travel is necessary for a medical emergency, you may contact the U.S. State Department Passport Duty Officer at (202) 647-4000. Please note the Passport Duty Officer will not be able to assist you if your contact is for business or pleasure travel. Normal processing time for passports is five weeks. If you need expedited services for business or pleasure, you may receive your passport within seven-to-ten days for an additional fee. For further information, please refer to the State Department Web Page http://travel.state.gov/passport/index.html or the National Passport Information Center at 1-877-487-2778.  Live operators stand ready to assist you from 8am-8pm EST.  It is automated 24 hours a day/seven days a week.  This is a toll free call.
